I *think* the reason you're getting the error message is that you're not logged into the message board, you have to be logged in in order to see the forms page.

Negative. I was logged in trying to view the form.

FWIW, another email was just sent out by the CMRA, and that pdf file actually worked/is downloadable. Check your emails in case there are other people besides me who were having problems.

For those of you saying "but I can't see meeeeeeeeeeee" and "the image is so small" - you know who you are ;), that was done intentionally to avoid people taking what they haven't paid for. That said, the image is rather small and with the watermark on it, it does make it difficult to see details [though that was part of the point, after all, if you remember saying "cheese" - your smiling face will be in the photograph!!]. Hopefully this version will make [some of] you happier:

a little less watermarking might give us a hint what we may be getting.
honestly, I woulda preferred to pay someone for the rights and offer a free download to all.
